Submitting an application does not automatically authorise or release information.

Before you begin

This process is for appointed Guardians and Custodians.

A Release Application is used to request access to information a Lyff subscriber has specifically designated for release following incapacity or passing.

It does not provide access to the subscriber's entire vault and does not include information the subscriber has kept Private.

Before applying, check that:
You are an appointed Guardian or Custodian You have been invited by the Lyff subscriber and have accepted that trusted role.
Your identity can be verified Lyff may require you to complete the applicable identity-verification process.
You are applying for Incapacity or Passing The release process is based on the circumstances selected by the subscriber for designated items.
You can provide supporting evidence Documentation is required before Lyff can assess a Release Application.

Why are you requesting access?

The supporting information required depends on whether the application relates to incapacity or passing.

Incapacity

Use this pathway where the subscriber is unable to manage their affairs and you are requesting access to information they designated for incapacity.

Medical evidence may be required.
An applicable tribunal or court order may also be relevant.
Lyff reviews the documentation and applicable release instructions.
Only designated information can be considered for release.

Passing

Use this pathway following the subscriber's passing where you are requesting access to information they designated for release in those circumstances.

Evidence of passing is required.
Australian or applicable overseas documentation may be considered.

Your details Your name, contact information and whether you are applying as the subscriber's Guardian or Custodian.

The subscriber's details The Lyff subscriber's full name and identifying account information requested in the application.

For an Incapacity application Relevant supporting evidence, such as a current medical certificate or applicable certified tribunal or court documentation.

For a Passing application An applicable death certificate or equivalent official documentation. Overseas documents may require an appropriate translation.

Your declaration You'll be asked to confirm that the information provided is true and complete and acknowledge the terms of the application process.

Lyff will review the application, the applicant's status, applicable verification requirements, supporting documentation and the subscriber's designated permissions before determining whether the release process can proceed.

Designed around the subscriber's choices

A release is limited by design.

The Release Application process does not convert a Guardian or Custodian into the owner of the subscriber's account.

Only information designated by the subscriber for the applicable release pathway can be considered.
Private items remain outside Guardian, Custodian and event-release access.
A Release Application does not provide general or automatic access to the subscriber's vault.
Lyff's role is administrative and procedural. Lyff does not make medical diagnoses or determine legal capacity.
Lyff does not adjudicate family, ownership or other legal disputes through the Release Application process.
Lyff may pause or decline a release where additional verification is required or where security, fraud, legal or privacy concerns arise.
